Eight correctional services officials face possible suspension after convicted prisoner Edward Khomotso Madiba escaped from Rooigrond Maximum Correctional Centre in Mahikeng.

Eight Correctional Services officials are facing possible suspension following the escape of an inmate from Rooigrond Maximum Correctional Centre in Mahikeng, North West.
Edward Khomotso Madiba escaped from the facility on Sunday, where he was serving a 20-year sentence for robbery with aggravating circumstances, possession of a firearm, kidnapping and parole violation.
The latest escape is the second time Madiba has broken out of the same correctional facility.
Correctional Services spokesperson Singabakho Nxumalo said an investigation was under way to establish how Madiba planned and executed the escape.

“A total of eight officials have been served with notices of contemplation of suspension, pending the outcome of the investigation into the circumstances surrounding the escape,” Nxumalo said.
Madiba Escaped From Same Prison Before
Madiba previously escaped from Rooigrond in 2022 and remained at large for about three years before he was recaptured last year.
His latest escape has raised questions about security at the facility and how he was able to leave despite being held in a maximum-security environment.

According to preliminary information, Madiba allegedly cut through the burglar bars of his prison cell before finding a way out of the facility.
The details of how he moved beyond the cell and ultimately escaped remain subject to the investigation.
Eight Officials Face Disciplinary Action
The Department of Correctional Services has launched an investigation into the circumstances surrounding the latest escape.
The eight officials who have received notices of contemplation of suspension will remain subject to the outcome of that process.
The notices do not amount to findings of wrongdoing, and the officials have not been found guilty of any misconduct.
Authorities are expected to examine the prison's security procedures, the circumstances surrounding the alleged cutting of the cell bars and whether any officials failed to carry out their duties.
Madiba remains a wanted escapee, while law enforcement agencies continue efforts to locate him.
